I waive and will not bring any claim against NCSD, including negligent acts of district agents, for damage to my personal property or any injury to myself as a result of my volunteer activity. I agree to this action to the fullest extent allowed by law. Applicants that fail to complete information provide misrepresentations or untrue facts will be denied the opportunity to volunteer. Per NCSD policy/standard practice, in an effort to maintain a safe environment, criminal history verification is required for some volunteer activities.

The North Clackamas School District is required to deny volunteer privileges to anyone convicted of certain felonies and misdemeanors (those listed in ORS.342.142). In addition the District reserves the right to deny volunteer privileges to persons based on the nature and recentness of crimes and overall criminal record. The Public Safety Department or the school Principal may revoke a volunteer’s privileges at any time if the volunteer’s presence is considered disruptive or unsafe to the learning environment.

North Clackamas School District strives to ensure a safe learning environment for our children. Therefore, any person that refuses a criminal background check will not be allowed to volunteer. There are no exceptions.

Volunteer Code of Confidentiality

North Clackamas School District is committed to maintaining the security and confidentiality of all student records and information. Selected volunteers with access to student records or information must adhere to the Volunteer Code of Confidentiality as outlined in the guidelines below.

  1. All student records should be considered confidential.
  2. Directory information, including a student’s name, address, telephone number, date and place of birth, photograph, major field of study, participation in officially recognized activities and sports, weight and height of members of athletic teams, dates of attendance, degrees and awards received and previous educational agencies or institutions attended, can only be shared with administrative approval.
  3. Records should not be left in a place where others can view them.
  4. Copies of records can only be shared with administrative approval.
  5. Volunteers should not discuss or repeat information overheard while in the staff lounge, classrooms, offices, school grounds, hallways, or school or extra curricular activities.
  6. Volunteers should not discuss information obtained while in a classroom, such as a student’s grade or behavior, with anyone other than the student’s teacher.
  7. Concerns or questions regarding student records or issues of confidentiality should be brought to the attention of the staff member that supervises the volunteer, and /or school administrator.
  8. Any knowledge of a violation of this Code of Confidentiality should be immediately reported to the staff member that supervises the volunteer, and/or school administrator.

Volunteer Liability Release

My participation in any volunteering activity is voluntary. I understand volunteering with District means that no compensation is expected in return for the services I provide, and the District will not provide any benefits typically associated with employment. I further acknowledge I am responsible for my own insurance coverage in the event of illness or personal injury as a result of my volunteering for District.

I understand my volunteering with District may involve activities that could be hazardous to me, including those which may expose me to communicable diseases. I fully understand and appreciate these risks that are inherent to my volunteering. I assume the risk of all bodily injury, medical treatment, illness, and/or death that may result from my volunteering for District, even if it results from the District’s negligence or that of its employees or agents.

I hereby release, waive, discharge, exonerate, and agree to indemnify and hold harmless District, its Board of Directors, the individual members thereof, and all officers, agents, employees, and representatives from any and all liability, causes of action, claims, demands, damages, expenses and compensation, including attorneys’ fees, fines or other costs arising out of arising out of injuries of any kind to me or to my property, or losses of any kind, including illness or injury from a communicable disease, which may result from or have any connection to my volunteering. I give this release to the fullest extent of the law, for myself, and my heirs, administrators, executors, successors and/or assignees. I further certify and represent that I have the legal authority to enter into this Agreement.

In the event that I require emergency medical treatment while participating in the Activity, I authorize the District and its agents to secure the help of a medical services provider and to incur the expenses for medical services recommended by the medical services provider. I agree to provide for the payment of these expenses.

This Agreement is intended to be as broad and inclusive as is permitted by law. If any provision or any part of any provision of this Agreement is held to be invalid or legally unenforceable for any reason, the remainder of this Agreement shall not be affected thereby and shall remain valid and fully enforceable.

I certify that I have read this document and fully understand its contents. I have read this document in its entirety and I freely and voluntarily assume all risks of such hazards and notwithstanding such, I agree to participate in any volunteering activity.
