To protect the safety and welfare of children, Siuslaw School District conducts criminal history background checks on all volunteers.

Failure to disclose convictions will result in automatic denial. A criminal record does not automatically disqualify an applicant, cases will be evaluated individually.

Your information will be submitted to Criminal Information Services INC (CRIS) after we receive it. This is a secure site and all information will be handled confidentially.

Your information will be submitted to local law enforcement agencies to determine other contacts with law enforcement and verification of CRIS reporting

  2. I will not repeat/disclose any private (health, grade or family) information received in my capacity as a volunteer.
  3. I will follow the instructions of the appropriate Siuslaw staff member on duty.
  4. I understand that discipline issues must be handled by a Siuslaw staff member.
  5. I will complete a Background Check form and provide official state issued photo identification.
  6. I will complete a drivers/insurance form if driving students in my personal vehicle.
  7. I have read the Volunteer Handbook and understand the information presented.
